Timed intercourse is natural conception guided by expert supervision. Most couples trying for a baby do not get it right in the first attempt. How do you know if you’re ovulating?

Indications of Ovulations:

LH Surge: occurs just before ovulation and triggers the rupture and release. Typically ovulation occurs within 48 hours of LH surge.
Follicle size: >16 mm
Body temprature: increases by 1 degree caused by increased levels of Progesterone
Middle Mittelschmerz pain: localized to one or the other side of the pelvis
Cervical Mucus thinning: induced by increase in Progesterone

Whom do we recommend timed intercourse?

Most couples under the age of 30, both in good health with no known infertility causes. Couples who have planning for pregnancy or who have recently started trying and need dome guidance. Usually the husband semen parameters are tested to be normal.

What happens in Timed Intercourse?

We start seeing the patient from Day 2 of period. We prescribe medicines (same a stimulation medicines?) That the intended mother would take daily till Day 9. From Day 9 follicular study is done to watch the follicular development and based on that we advise the couple to have Intercourse. Once the follicles size has reached 14 mm we perform a LH kit test to detect a sudden spike in LH hormone levels which happens 36 hours before ovulation. When the follicles have reached an appropriate size of >16mm on alternate days. By Day 13-Day 15 in 90% of cases this desired follicle size is achieved after which rupture and ovulation happens.

Why should you have TI on alternate days?

Ovum remains in uterus for 12-24 hours after ovulation. Sperm can survive from 24-48 hours. Intercourse is recommended in alternate days to increase the chances of fertilization. Let’s say we suggested first TI on the expected date of ovulation. This is day 0 of ovulation. If however the ovulation did not occur on Day 0 the sperm would still remain for 24-48 hours till Day 2. If the ovulation occurred on Day 1 or day 0. Another TI on Day 2 would ensure there are enough sperms for fertilization to occur.

Does recommending Timed Intercourse really help the infertile couple?

Natural conception success rate is 20-25%.
Timed Intercourse doesn't increase the success rate.
How it does help is that timing of the intercourse is programmed to the best possible time so that you do not miss the ovulation.
